Any student interested in participating in a sport of activity, please complete the required steps.
For Athletes Only. All others skip to step 2.
Step 1: Athletes must have a current sports qualifying physical exam on file in the Activities Office. Physicals are good for 3 years from exact date of last physical. Download a physical form at the Minnesota State High School League site at: http://www.mshsl.org/mshsl/publications/code/forms/PhysicalExam.pdf.
Once you have your physical completed by a doctor, you can turn it in to the office or you can scan it and submit completed Physical Forms online.
